The Senior Wellness Pharmacist of SWFL provides personalized consultations to help older adults and caregivers better understand prescriptions, over-the-counter medications, vitamins, and supplements. Choose a one-time review, additional follow-up support, or a focused vaccine review.
How to request an appointment:
Choose a service below and submit your preferred appointment time through Practice Better. I’ll review your request and send the required forms and payment instructions. Your appointment is confirmed only after the required paperwork and payment are complete and you receive a confirmation email.
Comprehensive Medication Wellness Consultation — Telehealth
$125 | 60–75 minutes
Get a thorough medication review from the comfort of home. Together, we’ll discuss what you take, your questions, and concerns such as possible side effects, interactions, duplicate medications, or difficulty following your medication routine.
You’ll receive a personalized Senior Wellness Medication Blueprint containing your medication list, medication-related observations, practical action steps, and questions to discuss with your healthcare providers.
Includes:
Review of prescriptions, OTC medications, vitamins, herbs, and supplements.
Personalized education for you and an authorized caregiver, if desired.
Your Medication Blueprint, delivered within five business days after the consultation and receipt of all necessary information.
One round of brief clarification questions within 14 days after Blueprint delivery.
Telehealth appointments are available to clients physically located in Florida.

Comprehensive Medication Wellness Consultation — In Home
$150 | 60–75 minutes
Bring your medication questions to a personalized consultation in your own home or a mutually agreed private location within the approved Southwest Florida service area. This service includes everything in the telehealth consultation, plus an opportunity to review your medication bottles, labels, storage, and organization together.
We’ll explore practical ways to make your medication routine easier to understand and follow. When appropriate, we may also discuss immunizations, medication-related fall risk, and other medication safety considerations.
Includes:
A comprehensive medication review and personalized Medication Blueprint.
Review of available medication bottles and labels.
Assessment of medication storage, organization, and adherence tools.
Education for you and your authorized caregiver.
One round of brief clarification questions within 14 days after Blueprint delivery.
Available within the approved Southwest Florida service area. Any additional travel fee will be quoted and agreed upon before booking is confirmed.

Medication Optimization Program
$375 | Three consultations within 180 days of the initial consultation.
For clients who want follow-up support after their initial medication review, this program provides time to revisit questions, discuss medication-related developments, and review progress on recommendations discussed with their healthcare providers.
Start with a comprehensive in-home consultation, followed by two telehealth appointments. Your Medication Blueprint will be updated based on information reviewed during follow-up.
Includes:
One initial 60–75-minute in-home consultation.
Two follow-up telehealth consultations, with 75 minutes reserved for each consultation.
An initial Medication Blueprint and updates based on follow-up information.
Personalized education for you and your authorized caregiver.
Brief clarification questions as described in your service agreement.
Program visits are completed within 180 days of the initial consultation unless an extension is agreed upon in writing.

Personalized Vaccine Review
$50 | 30-minute telehealth consultation
Unsure which vaccines you’ve received or what you may need next? This focused consultation reviews your available vaccination records alongside relevant health history, medications, and allergies.
You’ll receive a personalized written vaccine summary identifying potential vaccine needs, timing considerations, and questions to discuss with your physician or administering pharmacist.
Includes:
Review of available vaccination records and relevant health information.
Identification of potential vaccine gaps and items requiring clarification.
A written vaccine summary delivered within five business days after the consultation and receipt of all necessary information.
One round of brief clarification questions within 14 days after summary delivery.
Please submit available vaccination records before your appointment. This service does not include a comprehensive medication review, obtaining missing records from outside providers, vaccine administration, or vaccine costs. Recommendations reflect available information and guidance current on the review date.

Services are private-pay consultations and do not include diagnosis, prescribing, medication administration, or emergency care. Recommendations support discussion with your healthcare providers. Do not start, stop, or change prescription medications without consulting the appropriate prescriber.
